VETERINARY FORMS
 
The following Veterinary Forms are used during events for the management and the authorisation of emergency treatments, altrenogest use in mares, the use of cyclosporine ocular implants, cyclosporine topical ophthalmic medication, and the use of other medications and/or supportive therapies.
 
All treatment must be given within 'Designated Treatment Boxes' at events. 
 
Veterinary Form 1
Authorisation for Emergency Treatment (limited to Controlled Medication only). This form may be completed by an FEI Permitted Treating Veterinarian at an event or by the horse's usual veterinarian if the horse has received emergency treatment prior to an event. It must be presented to the event's FEI Veterinary Delegate immediately on arrival at the event.  
 
Veterinary Form 2
Declaration for administration of altrenogest to mares, the use of cyclosporine ocular implants and cyclosporine topical ophthalmic medication. This form may be completed by the horse's PR, the horse's usual veterinarian or an FEI Permitted Treating Veterinarian. It must be given to the event's FEI Veterinary Delegate immediately on arrival at the event.    
 
Veterinary Form 3
Authorisation for the use of medication and supportive therapies that are not included on the Equine Prohibited Substances List (for example: rehydration fluids and antibiotics) but which need to be monitored. This form must be completed by one of the event's FEI Permitted Treating Veterinarians. 
 
Veterinary Form 4
Self declaration for administration of specifically stated substances only, as described below in these VRs, but about which the FEI Official Veterinarians need to be informed. This form must be completed by the event's FEI Permitted Treating Veterinarian.
